New Medical Plan ID Cards

All members enrolled in the MMEHT health plans recently received new Anthem identification (ID) cards reflecting the January 1, 2018 benefit changes. You should have received a new ID card for each person covered under your medical plan. Please review the cards carefully to ensure we have the correct Primary Care Physician listed and that names are spelled correctly. Once you receive your new ID cards, please throw out your old cards and begin using the new ones immediately.  There is no need to call the Trust to set up or activate your 2018 coverage.

In addition, the look of the ID cards has been re-designed with color and larger fonts to make it easier to read and to find key information on the card.

Dental Plan ID Cards

Dental cards are not being reissued for 2018.  Please utilize your current Northeast Delta Dental card.  If you need an additional dental card, you may print one on the Northeast Delta Dental website at  As a reminder, ID cards are not issued for each individual enrolled in your dental plan.  All members may utilize the card under the subscriber’s name to access dental benefits.
