Long Term Disability Plan . . .

Employers with 10 or more eligible employees may be eligible to enroll in the Health Trust’s Long Term Disability (LTD) plan. This is a fully insured UnumProvident plan, which provides income benefits to employees who are unable to work for an extended period of time due to an accident, injury, or illness.

The LTD program will pay benefits after the employee has completed a 364-day elimination period. This elimination period corresponds to the 52-week maximum benefit period employees may be eligible to receive benefits under the Health Trust’s Income Protection Plan (IPP).

Eligible employees may receive an LTD benefit equal to 60% of the employee’s monthly earnings, to a maximum of $6,000 per month. This benefit may continue to age 65, or normal Social Security retirement age.
