Point of Service Plans . . .

The Health Trust offers three Point of Service (POS) plans to its members. These plans are administered by Anthem Blue Cross and Blue Shield. The Health Trust contracts with Anthem to provide access to its networks, and to process claims for Health Trust participants enrolled in the POS plans.

If you are covered under one of the Health Trust’s Point of Service plans, and you have questions regarding your coverage, please note the following telephone numbers.

For Point of Service benefit questions and claims payment, you should call the Health Trust Service Representatives at 1-800-852-8300, Monday through Friday from 8:00 a.m. to 4:30 p.m. Use Anthem's online Provider Directory to local in-network health care providers.

For Plan eligibility questions, you should call the Health Trust Billing and Enrollment Department at 1-800-452-8786 Monday through Friday from 8:00 a.m. to 4:30 p.m.

For Prescription Drug Card questions, you should call the Health Trust Service Representatives at 1-800-852-8300 Monday through Friday from 8:00 a.m. to 4:30 p.m.
