Finding a Primary Care Physician on Anthem’s Website

    Go to Anthem’s Website, at www.anthem.com.  Then click on these links:

• Members

• Select a State (Maine)

• “Find a Doctor”

• Provider Location

• Click on the “radio” button of the state you want (Maine)

• You can either search by location or lookup by name
  

Lookup by Name:   

Step 1: Enter Name

Type in the correct spelling of the last name of the Primary Care Physician (It is not necessary to enter the first name or Practice name)

Step 2: Select a Plan

             Click on the drop down box and select HMO Choice

Step 3:  Select Provide Type

             Click on the drop down box and select Physicians

Step 4:  Select Specialty

              Click on the drop down box and select Family Practice, Pediatrician, etc.

Click Next

Enter Address

• You only need to type in the name of the city/town where the PCP is located. You can select a mileage radius of 2, 5, 10, 20, 50, and 100 miles.  If you do not specify, the system will automatically select 20 miles.

• Scroll to the bottom of the page and click on “View Results”

Click Search Results

• Scroll down to the middle of the page and click on the name of the PCP that you are interested in.

Provider Detail

• This information provides the full name, address, phone number, education, hospital affiliation, whether the PCP is male or female and also if they are accepting new patients.

This page also provides the PCP ID number that is required when completing the Group

Health Enrollment Application.  The PCP ID number is found under the “HMO Choice”

header, on the first line (labeled “specialty”).  Please be sure to enter your PCP’s name

 and ID number on your enrollment form.
